Registration comprises two steps. First, you should fill out the online registration form for the workshop. Then you should email us payment information as explained below.
Participant registration is guaranteed only when the registration fee has been paid.
We strongly prefer that all payments be made by credit card. Please download and fill out the workshop payment form then return it by email to Dakota Nanton (email@example.com). Please include "FSM2017" in the subject line of your email.
Unfortunately, some web browsers (including Firefox and Internet Explorer) do not support fillable forms. In this case, you could try opening the payment form in a different browser (Chrome or Safari). Once you have saved the form to your hard drive (you will need to do this eventually), you should be able to open this PDF document using Acrobat Reader in order to edit and save your payment information.
If you absolutely cannot pay using a credit card, a check made out to "University of Colorado" should be mailed with the name of the registrant and the conference ("FSM 2017") noted on the check, made out in U.S. dollars and drawn on a U.S. bank, to:

The University of Colorado Boulder cannot accept purchase orders.
For your convenience, a confirmation message will be emailed to you once your payment has been processed. We recommend that you print this email for your records. If you do not receive confirmation of your payment within three business days after you email your payment information, please contact Dakota Nanton (firstname.lastname@example.org).
If you need to make a change once you have received your confirmation, please send an email with your name, city/town, and your requested change(s) to email@example.com . To avoid duplicate registration and additional charges to your account, please do not register a second time.
Cancellations and Refunds
If you must cancel your registration for the workshop, please email your written cancellation notice by August 21, 2017 to firstname.lastname@example.org. All cancellations are subject to a $35 processing fee. No refunds will be provided after August 21, 2017.
You may register on-site starting at 8am on Thursday 21 September.
